The following link will bring you to some Recording forms that are a practical way to keep up with a particular behavior (s). We understand that it can be very time consuming and difficult to monitor and record a child's behavior when you may have a classroom full of students who have their own needs. However, these Recording Sheets allow you to observe within short increments of time throughout the day at your convenience. You may be looking for the magnitude of a behavior, the duration of behavior, the frequency, or the time of day that it most likely occurs, and these Recording forms can be a helpful way to keep up with each of those issues. Think of them as a checklist, so that once you get an idea of what's specifically going on, you can then formulate a strategy to reduce unwanted behavior and increase a desired behavior.
